Social Service and discharge management
Both an illness and the resulting clinic stay can cause short or long-term changes in a variety of spheres of the patient’s life. The personnel from our Social Service will advise and support you in organising specific help services for the period immediately after your discharge.
The main areas of activity are:
- Rehabilitation measures
- Day, short-term and inpatient care
- Care at home
- Provision of care aids
Naturally, we also offer you a discharge management service where required (in accordance with the stipulations of Section 39 Para. 1a S. 9 SGB V).
