Penile corporoplasty (surgery to straighten the penis)

A penis straightening operation (penile corporoplasty) is a possible option in the case of congenital or acquired penile curvature.

Congenital curvature of the penis is a pronounced curvature of the penis, usually downward or sideways, which patients usually become aware of during puberty.

The acquired curvature of the penis can be caused by the disease induratio penis plastica (IPP). IPP (also known as Peyronie's disease) is an inflammatory disease that typically occurs in two phases, an inflammatory phase (duration without treatment: 1 - 1.5 years) and then a post-inflammatory stable phase. During the inflammatory phase, one or more zones of cartilage-like hardening (plaque) form in the skin of the corpus cavernosum (tunica albuginea), which therefore completely loses elasticity in those areas.

Therapy of the inflammatory phase of the disease

There are a variety of therapeutic procedures for treating the inflammatory phase of IPP, all of which aim to stop the inflammatory process and hence the further progression of the disease (especially penile deformation).

Orally administered drugs are used as a conservative therapy.

Semi-conservative therapy approaches include local radiation therapy on the plaques using low-dose radiation therapy, injection of various drugs into the plaques, and local shock wave therapy (ESWT).

Therapy of the post-inflammatory stable phase

Once the inflammation has ceased and the condition is stable, therapy for the penile deformity may be attempted by injecting collagenase into the plaque. There are different variants and techniques of so-called penile corporoplasty. However, not every surgical technique seems to be appropriate for every patient and their condition.

The strategy of Prof. Dr. med. J. Ullrich Schwarzer of using primarily autologous material and no foreign material for penile corporoplasty is based on his long clinical experience with more than 700 performed penile corporoplasties.

Our experience with relapse surgery shows that foreign materials can induce tissue hardening of the corpus cavernosum and thus, similar to the underlying induratio penis plastica (IPP), cause a loss of elasticity of the corpus cavernosum. For this reason, we primarily use the body's own patch material and only use a collagen fleece if the curvature is very pronounced and the body's own patch material available is not large enough.
