Clarifying consultation

Anaesthesiology

A few days before the operation, you will attend a clarifying consultation at our premedication department for outpatients. This allows the anaesthetist to form an impression of your health status and he will also clarify the necessary measures as well as the risks of the anaesthetic with you.

You can arrange an appointment for a clarifying consultation by calling our patient management department (Tel: +49 (0) 89 85693-2512). Once you have made an appointment, you will be sent a number of questionnaires which you should fill in and bring along with you to the consultation.

Please bring the following documents with you when you attend the clarifying consultation:

  • Health insurance card and any supplemental insurance card
  • Hospital referral note (pink form)
  • Letter of referral from the referring doctor (yellow form)
  • Medication regimen
  • Information about pre-existing conditions
  • If available:
    • Current medical reports
    • Pacemaker ID
    • Allergy pass
    • Anaesthetic passport
    • Patient decree (further information)
  • Completed anaesthetic form and coagulation questionnaire (this will be sent to you)
  • Completed questionnaire on hospital hygiene (this will be sent to you)
  • For patients under a guardianship, we need a guardian to clarify matters regarding the anaesthesia (please bring guardianship document or care directive with you)

In addition to the anaesthesia clarification, a urological clarification will also be conducted along with any required preliminary examinations.
